Yes and no. It depends on the size of the guy's penis and how they both use their genitals. If she has a tight vagina and he has a huge penis, chances are, it's going to hurt her. If it's the opposite, it may not be as pleasurable an experience . . . unless they know what they're doing. (Both women and men can tighten their muscles in their genitals together to create a wonderful experience.)
Some women find that after childbirth, their vaginas are loose. There are exercises called Kegels that women can do to tighten those muscles again.
No, it does not except for those guys who have a very small penis.
