Jewelry in that area of the body is quite subject to infection. Plus that area of the body is meant for use. Stretching skin is not a good place for a hole to be placed.
Stick to decorative jewelry. Preferably not in a place full of bacteria.

There is NO possible way to keep it completely germ-free...ever.
You'd deal with a vicious battle of infections/healing/and scaring. Eventually only scar tissue would be built up and the tissue will have limited stretching.
